By using the telehealth services available through slimdoor.com (“Site”), operated by Slimdoor LLC (“Company,” “we,” “us,” or “our”), you acknowledge and agree to the following terms regarding telehealth consultations and services (“Telehealth Services”).
1. Nature of Telehealth Services
Telehealth Services involve the use of electronic communications, including but not limited to secure messaging, asynchronous questionnaires, and video or audio consultations, to enable healthcare providers to evaluate, diagnose, and treat patients at a distance. 2. Provider Relationship
By using the Telehealth Services, you understand and agree that:

- There is no guarantee that a prescription will be issued. The decision to prescribe is made solely in the professional judgment of the treating provider.

3. Risks and Limitations
You understand and acknowledge the following risks and limitations of Telehealth Services:
- Telehealth consultations have limitations compared to in-person visits, including the inability to perform a physical examination.
- The provider will rely on information you provide, including medical history, current symptoms, and photographs. Incomplete or inaccurate information may affect the quality of care.
- Electronic communications may be subject to technical failures, interruptions, or delays. The Company is not responsible for disruptions in service caused by technology failures.
- Although reasonable safeguards are employed, electronic communications carry inherent security risks. You accept these risks by participating in Telehealth Services.
- In rare cases, information transmitted may be insufficient to allow for appropriate medical decision-making.
4. Your Responsibilities
By consenting to Telehealth Services, you agree to:
- Provide accurate, complete, and current health information to the best of your ability.
- Promptly inform your provider of any changes in your health condition.
- Follow the treatment plan prescribed by your provider, including medication instructions and follow-up care.
- Seek emergency care immediately if you experience a medical emergency. Telehealth Services are not a substitute for emergency medical care.
- Notify the Company of any adverse reactions, side effects, or concerns related to your treatment.
5. Prescription Medications
If your provider determines that prescription medication is appropriate:
- The prescription will be transmitted electronically to a licensed pharmacy for fulfillment, which may include 503A or 503B compounding pharmacies.
- You understand that compounded medications are not FDA-approved but are prepared by licensed pharmacies in accordance with applicable regulations.
- You are responsible for using the medication as directed by your provider and for reporting any adverse effects.
- Refills and subscription renewals may require periodic provider review.
6. Privacy and Confidentiality
Your health information will be handled in accordance with our Privacy Policy, HIPAA Authorization, and applicable federal and state privacy laws. By consenting to Telehealth Services, you authorize the Company to share your health information with the treating provider, fulfilling pharmacy, and other parties as described in our HIPAA Authorization.
7. Payment and Refund Terms
Payment for Telehealth Services is collected at checkout and is subject to the Company’s Shipping, Refund & Payment Policy. By completing your purchase, you expressly authorize payment for services initiated at checkout, including medical consultation and evaluation, and agree to all applicable Terms of Use, Telehealth Consent, and Refund Policy. You acknowledge that services begin immediately upon submission of your medical intake and are not contingent on prescription approval. The consultation and evaluation fee is non-refundable once the medical intake has been initiated, as the medical service has commenced regardless of outcome. Medical consultation and review services are considered rendered upon submission and review of your medical intake, regardless of whether a prescription is ultimately issued. If a prescription is not approved by the provider, the medication portion of your payment may be refunded; the consultation portion is retained. Once a prescription has been approved and transmitted to the pharmacy, no refunds will be issued except where required by applicable law. Once an order has shipped, no refunds, returns, or exchanges will be processed except where required by applicable law.
8. No Guarantee of Treatment or Outcome

9. Emergency Situations
**TELEHEALTH SERVICES ARE NOT INTENDED FOR MEDICAL EMERGENCIES. IF YOU ARE EXPERIENCING A MEDICAL EMERGENCY, CALL 911 OR GO TO YOUR NEAREST EMERGENCY ROOM IMMEDIATELY. DO NOT RELY ON TELEHEALTH SERVICES FOR URGENT OR EMERGENCY CARE.**
10. Right to Withdraw Consent
You may withdraw your consent to Telehealth Services at any time by notifying the Company in writing. Withdrawal of consent will not affect the validity of any actions taken prior to receipt of your withdrawal. Withdrawal of consent does not entitle you to a refund for services already rendered.
11. Governing Law
This Telehealth Informed Consent is governed by the laws of the State of Georgia and is subject to the dispute resolution and arbitration provisions set forth in the Company’s Terms of Use.
12. Acknowledgment
By proceeding with a telehealth consultation through the Site, you confirm that:
- You have read and understand this Telehealth Informed Consent.
- You voluntarily consent to receive Telehealth Services.
- You understand the risks, limitations, and conditions described herein.
- You are at least 18 years of age.
- You are located in the United States at the time of your consultation.
- You understand that payment is collected at checkout and is not contingent on prescription approval.
